Position Summary

The Aged Care Personal Care Worker provides care, supervision and support for aged people in the day-care centre. Aged Care Personal Care Worker assists with the maintenance of personal care, domestic duties and management of illness. They also provide companionship and emotional support and promote independence and community participation. The Aged Care Personal Care Worker may assist with mentoring less qualified aged care personal care workers.

Core Duties & Responsibilities include:

  • Provide support to the clients of the service, these include;
  • Showering, lifting, moving, dressing, grooming, exercising and feeding clients,
  • Assist with communication and advocating for the client,
  • Identify the need for and support clients to attend their medical appointments,
  • Encourage positive and socially appropriate behaviour, and effectively manage,
  • Assist with the delivery of activities to enhance the physical, social, emotional, and intellectual development of clients in aged care services,
  • Transport clients to/from the Clinic/Day Centre to their home or destination of choice in the Yalata community as required,
  • Work with a team of health professionals, family, friends and carers to implement a programme of support.

Remote Area Nurse (Aged-Care Portfolio/Focus)

Position Summary

You will be joining a team of multi-disciplinary personnel, which includes Aboriginal health workers and clinical staff that aim to establish, maintain and provide primary health care services with the goal of achieving best practice health outcomes for all ages of Yalata community.  This position will have a focus and portfolio of aged-care clients.

Core Duties & Responsibilities include:

  • Participation in the after-hours “On Call” roster with other health staff
  • Assist and respond to roadside emergencies in cooperation with Ceduna Ambulance and Local Police.
  • Performing consultations with clients, including listening to their stories, providing a basic physical examination and consulting with other clinical staff as necessary, and advise on the treatment and management of clients’ health problems.
  • Assistance in client follow-up, checking of immunisation status, and when other activities are scheduled (e.g. PAP smears, chronic disease checks, STD checks).
  • Provide chronic disease management/care to the clients of the clinic with the aim of achieving minimal risk factors of the client health.
  • Recording in the details of the clinical notes after consultation, including history, examination and management offered and any follow-up organised; he/she will assist the client in organising referrals (including appointments) to other agencies as necessary)
  • Act as a clinical resource to support, mentor and develop Aboriginal Health Workers, students and other members of the team.
  • Assist in annually auditing the clinic to ensure we are meeting RACGP & Aged Care accredited standards.
